Longevity Diagnostics’ 4 Step Approach provides a road map for your health journey. You can take control, become informed, and make the changes to your diet and lifestyle that lead to a long and healthy life.

step 1
  • Select a Health Test Panel.
  • Fill in a short questionnaire about your current health status. The questionnaire will inform your risk assessment and your nutrition and lifestyle coaching session.
  • Read and accept the Longevity Diagnostics Privacy and Terms of Service Statement.
  • Complete the Longevity Diagnostics Payment Form.
  • Receive email confirmation of your purchase.

  • Receive the Test Requisition/Order Form for your blood test by email.
  • Schedule your blood draw appointment with the Quest Phlebotomy Site listed in the email.
  • Fast for at least 8 hours before blood draw, that is, nothing to eat or drink except for water, black coffee or tea without dairy, non-dairy, or sugar products added.
  • Take requisition to Quest Phlebotomy location for a blood draw.

  • Receive your Longevity Health Report by email.
  • Learn your blood levels for over 25 biomarkers of longevity.
  • Know your Longevity Score.
  • Understand your risk for cardiovascular disease (heart disease and stroke), cancer, lung disease, dementia, infections, kidney disease, diabetes, and liver disease, based on your health history and your test results.
  • Share your Report with your healthcare provider.

  • Receive email with link to Longevity Nutrition & Lifestyle Questionnaire and Coaching Calendar.
  • Complete and submit the questionnaire.
  • Schedule your 30-min, one-on-one Nutrition & Lifestyle Coaching teleconference.
  • Work with a Lifestyle Coach to identify priorities to lower your disease risk and promote your longevity.
  • Receive Longevity Nutrition and Lifestyle eBook and access to Living Plate menu planner with hundreds of kitchen-tested recipes.
  • Purchase additional Lifestyle Coaching Sessions for optimal results.
